CANCELLATION & REFUND POLICY
No call/no show: $100 fee. Same-day cancellations: first per year is waived, subsequent cancellations incur $100 fee. Late fees must be paid before next appointment. Prescription holds apply to unpaid late fees.
All patients must have a valid credit card or debit card on file. Payment is due at time of service. Prescriptions will be held for any unpaid balances including co-pays and fees. Outstanding balances must be paid before future appointments can be scheduled.
